Privacy Policy for Socially Determined's Public-Facing Web Services

Thank you for visiting Socially Determined’s public website or engaging with us online via social media services. At Socially Determined, we value your privacy and are committed to protecting your personal information.

This policy applies to all our public-facing web services where no login or personal information submission is required, including:

These platforms allow us to share informational content about SDOH, industry insights, and company updates, fostering engagement with healthcare professionals, partners, and the public. Interactions on these sites—such as likes, comments, shares, or direct messages—do not involve Protected Health Information (PHI) or require user login to our systems.

We do not collect or process PHI via these public channels. This policy is separate from our comprehensive Privacy Policy for SocialScape Platform, which handles PHI under HIPAA and our HITRUST certification. It complies with relevant data protection standards, including influences from CCPA/CPRA where applicable for U.S. residents, and is reviewed annually.

Information We Collect

We collect limited, non-personal information across our public-facing services to support operations and engagement. No personally identifiable information (PII) is required or collected unless voluntarily provided (e.g., in a comment or message). We collect personal information that you voluntarily provide to us, such as your name, email address, phone number, and any other information you choose to provide.

  • On Our Website (https://www.sociallydetermined.com):  We automatically gather device and browser information (e.g., browser type, version, operating system, device type, screen resolution), usage data (e.g., pages visited, time spent, referral sources, navigation patterns), network information (e.g., IP address, approximate geographic location at city/region level, internet service provider), and data via essential/analytics cookies. Tools like Google Analytics (anonymized) may be used. Additionally, we may collect certain information automatically when you visit our website, such as your IP address, browser type, referring/exit pages, and operating system.

  • On LinkedIn, X, and YouTube:
    • Platform-Provided Data: These third-party platforms collect data per their own policies (see LinkedIn's Privacy Policy at https://www.linkedin.com/legal/privacy-policy, X's at https://x.com/en/privacy, and Google's athttps://policies.google.com/privacy). We access aggregate analytics (e.g., post views, engagement metrics) without identifying individuals.
    • Interaction Data: If you interact with our posts (e.g., like, comment, share, or send a direct message), we may see public details like your username, profile information, and interaction content. We do not systematically collect or store this   for  profiling.
    • Voluntary Submissions: If you provide contact details or other information via messages or comments (e.g., inquiring about services), we treat it as voluntarily shared and handle it securely.

We do not collect sensitive data like PHI, financial details, or demographics on these platforms.

How We Use Your Information

Data is used solely for legitimate purposes tied to our public outreach or response to your interest in Socially Determined:

  • Providing and personalizing our services to meet your needs
  • Responding to your inquiries, questions, and requests
  • Sending you newsletters, marketing communications, and updates about our products and services
  • Conducting research and analysis to improve our website and services
  • Monitoring and preventing any unauthorized access or misuse of our website
  • Complying with legal obligations and industry standards
  • Social Media Uses: To monitor engagement (such as responding to comments or messages about SDOH topics), improve content (e.g., understanding popular posts via platform analytics), and for community building, such as following up on professional inquiries.
  • General Uses: Providing and personalizing our services to meet your needs; responding to your inquiries, questions, and requests; sending you newsletters, marketing communications, and updates about our products and services (with opt-out options); conducting research and analysis to improve our website and services; monitoring and preventing any unauthorized access or misuse.

No data is used for individual profiling. We adhere to the "minimum necessary" principle and anonymize where possible.

How We Share and Disclose Information

Sharing is minimal and focused on platform operations:

  • Website Sharing: With our service providers (hosting and analytics) under data processing agreements, or for legal requirements.
  • Social Media Sharing: Interactions occur on third-party platforms, so data is shared per their terms (for example, public comments are visible to others). We may share aggregate insights internally or with partners (such as anonymized engagement stats), but not individual data. If you provide information leading to a business relationship (for example via LinkedIn message), it may transition to our Customer Privacy Policy, which offers stricter protections for PHI and other information covered under a formal, mutually-executed customer agreement with Socially Determined.
  • Additional Disclosures: By visiting any of our public services you consent to our sharing your data with service providers for functions like website hosting, data analysis, and customer support; if required by law or in response to a valid legal request (a court order or government investigation); in the event of a merger, acquisition, or sale of assets (business transactions), where your information may be transferred to the acquiring entity.
  • No Sales: We do not sell or rent data.

Data Security

Our HITRUST certification extends the policies, procedures, technical controls and security principles applied to our SocialScape Platform (where PHI is stored and processed) to public web services:

  • Our Website: TLS 1.2 or higher encryption-in-transit, access controls, regular vulnerability scans
  • Social Media: We use secure platform features, including multi-factor authentication on our accounts, and we monitor for risks. However, security is also governed by LinkedIn, X, and Google policies—users should review those for platform-specific protections.
  • Incident Response: We post notifications on our public website if a breach affects data we control.

Cookies, Tracking, and Platform Technologies

We use cookies and similar tracking technologies to enhance your experience on our public-facing services. These help with basic functionality and anonymous analytics. Below, we explain the types of cookies, how we obtain consent, and your options to manage them.

  • Types of Cookies We Use:
    • Essential Cookies: These are necessary for the website to function properly, such as maintaining sessions, enabling navigation, and ensuring security. They do not require consent and cannot be disabled without impairing site usability.
    • Analytics Cookies: These collect anonymized data on usage patterns (e.g., via Google Analytics) to help us improve content and performance. They are optional and require your consent.

Consent Process: On your first visit to our website, a cookie consent banner will appear, providing clear information about cookie categories. HubSpot-powered sites like ours typically feature a cookie consent banner on your first visit, allowing initial choices.

  • Here is what the banner choices mean:
    • Accept: Opt in to all cookies (essential + analytics).
    • Decline: Opt out of non-essential cookies (analytics, advertisement), allowing only essentials.
    • Cookie Settings: Select specific categories (analytics, advertisement) to opt in or out granularly. We store your preferences for future visits but respect revocations immediately if you clear your cookies and reset your preferences or by contacting us.

  • How to Opt In or Out:
    • Via Consent Banner: Use the banner to manage choices easily.  If you wish to change your preferences, click the Cookie Preferences link in the footer of the Socially Determined website.
    • Browser Settings: Most browsers (e.g., Chrome, Firefox, Safari) allow you to block or delete cookies or enable the “Global Privacy . Go to your browser's privacy settings to set preferences—note that blocking essentials may break site features.  If you clear your browser cookies, the consent banner may reappear on your next visit.
    • Do Not Track (DNT) or Global Privacy Control (GPC) Signals: We honor DNT and GPC browser signals where feasible, automatically opting you out of analytics.
    • Specific Opt-Out Tools: For Google Analytics, install the official opt-out browser add-on at https://tools.google.com/dlpage/gaoptout. For broader controls, use extensions like uBlock Origin or Privacy Badger.
    • California Residents (CCPA/CPRA): If analytics sharing qualifies as a "sale," submit an opt-out request to ccpa@sociallydetermined.com—we'll process it without verification delays for cookies.  We review our cookie practices annually to ensure compliance with laws like GDPR and CCPA/CPRA.

  • Social Media Tracking: On LinkedIn, X, and YouTube these platforms use their own cookies and trackers (e.g., LinkedIn Insight Tag for engagement metrics, X pixels, or YouTube cookies for video views and user behavior). We may embed these for aggregate analytics but do not control them. Adjust settings directly on those platforms (e.g., LinkedIn's privacy settings, X's data preferences or YouTube's ad personalization settings).

Opting out does not affect your access to our content, and we do not discriminate against such choices.

Your Rights and Choices

You have certain rights and choices regarding your personal information:

  • Website: You may opt out of analytics; contact us to request access/deletion for voluntary data.
  • Social Media: You may control visibility via platform settings (e.g., private profiles on LinkedIn/X). You can unfollow, block, or delete interactions. For data we receive (such as messages), request review/deletion via privacy@sociallydetermined.com.
  • General: You may opt-out of receiving marketing communications from us, update your preferences, or request access, correction, or deletion of your personal information by contacting us at privacy@sociallydetermined.com. Broader rights under CCPA/CPRA apply (e.g., no "sales," non-discrimination, and other protections). Please direct platform-specific requests to LinkedIn, X, or Google.

California Consumer Privacy Act (CCPA - Opt-Out Rights

If you are a California resident, you have the right under the California Consumer Privacy Act (CCPA) to opt-out of the sale of your personal information. We do not sell personal information to third parties for monetary value. However, certain sharing of personal information as described earlier may be considered a "sale" under the CCPA.

To exercise your right to opt-out, please submit a formal inquiry to ccpa@sociallydetermined.com. Please note that we may need to verify your identity before processing your opt-out request. We will respond to your request within the timeframe required by law. Please be aware that opting out does not mean you will no longer see advertising. You may still receive generic ads that are not based on your personal information.

We do not have control over, and assume no responsibility for the content, privacy policies, or practices of any third-party sites or services.

Data Retention

  • Website: Logs up to 30 days; analytics up to 26 months.
  • Social Media: We do not retain interaction data long-term; platform data is ephemeral unless voluntarily provided (retained as needed for follow-up, then deleted). Data is securely deleted when no longer needed.

Children's Privacy

Our public services are not for children under 13. We do not knowingly collect personal information from children under 13. If we become aware of such collection, we will take steps to delete it promptly.

International Users

Data processing occurs in the United States. If you are accessing our public-facing web services from the European Union (EU) or European Economic Area (EEA), the General Data Protection Regulation (GDPR) may apply to our processing of your personal data. As a US-based company, we comply with GDPR requirements where applicable, including ensuring lawful bases for processing (such as consent for analytics cookies), upholding your rights to access, rectify, erase, or restrict your data, and implementing appropriate safeguards for international data transfers, such as Standard Contractual Clauses (SCCs). Third-Party Platforms

Interactions on LinkedIn, X, and YouTube are primarily governed by their privacy policies. We encourage reviewing them. Our policy supplements by addressing how we handle any data received from those interactions.

Updates to our policy will be posted here (Version 3.1, Effective Date: July 16, 2025). Continued interaction with our services implies acceptance.
